  1. Structure around three questions: what went well, what didn't, what should we change.
  2. Focus on systems and processes, not individual blame.
  3. Leave with a small number of specific, owned action items — not a long unactioned list.
  4. Revisit previous retro action items at the start of the next one to close the loop.
